Finally I have my Int Med Cycle ended one week ago and finally I have time to update my blog. Let's talk about int med this semester. We have Natalia and Palovna as our teachers for this sem again, actually they are the only int med teachers for third year. Natalia is a young teacher and she spoke quite soft, we used to complaint about her in previous sem because she spoke softly and her teaching is always skipping from one topic to another topic, but actually she is a good teacher, she have the knowledge and tried to teach us. She was happy when we started to answer her question instead of keeping silence and she the one do all the talking and teaching part. So, we actually learnt that if we prepare well for class and try to answer teacher's question although we don't know or might it be wrong, but it doesn't matter, as long as we try and we think; By this manner, we could actually learn more than actually what teacher could teach, and teacher will be happy as she get the response and know that we actually did reading. University is not the same as secondary school anymore, teacher will not spoon feed you everything you need to know, they can just give up teaching and walk away if you never prepare for class. So, Tip #1 always prepare for class.
For Palovna, holy shit, the super fierce one, but believe me she is really pretty when she smile (not a sarcastic smile of course). She is an old modern lady, she actually do not look old at all, but we heard that she already have grandchild. She always scold us for not reading (with super loud voice), fierce and loud enough to chase away all the drowsiness and sleepiness haunting us. We all scared of her, and we all want her as your teacher (actually I want), cause a fierce teacher can always motivate me to study if I don't want to get scolded. But this sem, she seldom scold us, I think maybe we answer her question, although it might be wrong. She will criticise you if you give wrong answer but she will correct you; if you keep silence, she will think that you never prepare for class and stop teaching. So, Tip #2 never keep silence when teacher want to hear an answer, someone have to say something although your brain is all blank.
There was an incidence, Palovna asked us to train and revise our practical skill as we will have to prepare a case report of a patient and the end of the cycle. She want to check our practical skill like percussion and palpation of liver or spleen, auscultation of heart, etc as she always say that, "patient is not the one you practise on, your friend is the one for your practise!" On that day, she was late for class, and we all sitting there and wait her without doing anything. She scolded us that we should use the time for practising instead of sitting, so she started to ask about the practical skill, how to percuss liver, what is the kurlov's size of liver, and luckily we all managed to answer her, and this saved our life; her bad mood and anger soon gone once we all answered her with correct answer. She was happy that day and kept smiling. Tip #3 if you don't pretend to do anything during free time of class, you have to really know something.
At the end of the cycle, we paired up and went to patient for our case report. Okay, that was not an easy job, language is the real problem we faced. So end up we were not really get all the information we wanted. That's okay, case report can be "modified", and everything still based on the information we got from patient. We passed up the case report and had a conference to discuss our patient with Palovna. Of course we made a lot of mistakes about diagnosis, plan of investigation, ECG. But nevermind, we still got a satisfactory mark from her. But, There was a classmate, who got a patient with liver cirrhosis, based on her clerking and examination, the patient didn't really have characteristic that a liver cirrhosis patient should have and she might miss some vital signs that indicate liver cirrhosis. So, when Palovna asked about her patient, teacher doubted on her result of investigation, she told her that a liver cirrhosis patient must have jaundice, hepatomegaly, etc and starting to explain us again what findings we can see from a liver cirrhosis patient. However, that particular classmate, defenced her findings and insisted that the patient really did not have any findings that teacher told her. Okay, this was ridiculous, teacher was pissed and said that, "you don't see because you don't know; you have such an interesting case compared to other and you did not really investigate properly!" Of course, she got bad mark. So peeps, Tip #4 Never never never challenge a teacher (doctor) experience/ knowledge especially she have more than 20 years experience.
That's all I want to say about my internal medicine cycle this sem. Soon I will have my Internal Medicine exam in June, and examiners are all killers including Palovna. So, pray hard!!!
